A healthcare mandate is a Louisiana mandate that specifically gives an agent authority to make health-care decisions for the principal. It is useful when illness, injury, incapacity, or another circumstance prevents the principal from handling those decisions personally.
A healthcare mandate is different from a Living Will. The mandate names a decision-maker; a Living Will records directions about life-sustaining treatment in the circumstances covered by Louisiana law. Many clients choose to have both documents.

Before the appointment
You do not need legal language prepared before the meeting. Rouge will need accurate names and the decisions you want reflected in the mandate.
Have the full legal name and reliable contact information for the person you want to appoint.
Decide whether you want a backup person if the first agent cannot serve.
Bring any existing mandate, Living Will, or estate-plan documents you want Rouge to review for consistency.
Have current government-issued identification available for execution.
